INSURANCE: You agree to pay any portion of the charges not covered by insurance. It is the responsibility of the patient to inform the office of any changes to insurance and know his or her insurance benefits.
UNINSURED: Payment is due at the time of service. You may pay with cash, debit card, major credit card, or personal check.
PAYMENT AT TIME OF SERVICE: We ask any portion not covered by your insurance due to deductibles or co-pays be paid on the date of service.
RETURNED CHECKS: There is a minimum $35 fee for all returned checks.
FORMS & RECORDS: There is a $15-$25 fee for paper copies of medical records, disability forms, or forms with multiple pages requiring the provider complete for the patient. To receive your records, your balance must be paid in full.
If you have questions about your EOBs (Explanation of Benefits) or your bill from our office, please stop in or call with both your bill and EOB, and we will be happy to answer any question you might have. Please note that some questions may need to be directed to your insurance carrier.

Medication "TO DO" List
BEFORE YOU RUN OUT, please call 48 hours prior to needing refills.
DO KNOW the medications you take and cannot take. It is best if you keep a list with you.
DO REQUEST REFILLS at your office visit or before you run out, as it can take 24-48 hours.
DO NOT WAIT to call for refills until your medications have run out, or on weekends/Friday afternoons.
DO REQUEST a written prescription for mail-in refills.
